You missed an appointment. Write a letter explaining what hap- pened. In your letter include: *  what the appointment was
 *  a apology for missing the appointment
 *  a request to reschedule.

Dear, Sir I am writing
this
letter regarding the scheduled appointment for a health checkup at Lakeside clinic. As I was unable to present myself there. Allow me to brief my absence.

We all know that your services are excellent and the staff is very gentle toward the patients which cause high walking and long queue for appointments. my appointment was planned yesterday at 11:30 am for a full body checkup by Dr Yash, we use to take consultation and advice from him on a regular period of interval.

I would like to show sincere apologies for not noting my presence as per request because some of my guests arrived surprisingly at my stay .They came from far north and have been travelling for the
last
28 hrs. So attending them was a priority for me. Just to bring to your notice I have yearly membership
also
. I am looking for a new appointment at tomorrow on special request. Please let me know the confirmation on the same.

I hope to hear from you soon positively !

The Greeting

Depending on the style and aim of the letter, you will need to adapt your greeting.

Always start an informal letter in the ways:

  • Dear + name
  • Hi / Hello + name

‘Dear...’ is more appropriate, so stick with this.

For a formal letter there are two options for the greeting:

  • Use Dear Sir or Madam if you don’t know the name of the person you are writing to.
  • Use Dear + surname if you do know their name, e.g. Dear Mr Smith or Dear Mrs Jones.
